With a collaborative practice agreement, our pharmacists can provide essential over-the-counter products to help keep your little one healthy and treat common issues like allergies, fevers, or diaper rash. These items are available at no cost to you through your insurance and can be picked up right here at the pharmacy, no doctor’s visit required. It’s a simple way to get the care and support your child needs, when you need it!

How it Works

Parents/guardians can stop by our pharmacy with the child, and we will obtain your child’s medication history, weight, and a list of allergies. We may then offer the selected items on this form, written as a prescription
Items listed may be refilled once every 30 days
Parents/guardians can set up refills for monthly pickup or ask for refills as needed

Get Started

To begin the program, you must fill out the form linked to the red button below. You can drop it off at any of our pharmacies. Or, for your convenience, you can take a clear picture of the completed form and text the photo to the location you would like to use. Please note that a PDF will not go through text. It must be a photo. After the form is submitted, we will follow up with more information!
